We're glad you decided to join the Net Pay Family.  To get you set up and ready to run your payroll is easy.  Please download the forms to the right and follow the instructions below.  Once finished, please fax all forms back to Net Pay at (717) 236-5301.  If you need help with any of these forms, please feel free to contact us anytime Toll Free at (866) 463-8729.

Print the forms to the right and follow the directions below:

 New Client Setup Form 

The New Client Setup form contains information we need to know to set your account up with Net Pay. 

  1. Federal ID Number - Enter your IRS Tax EIN Number in this box
  2. State Income Tax Number - If your in a state that has Personal Income Tax, enter your ID number from your state in this box
  3. State Unemployment Tax Account Number - Enter your Unemployment Account number. Every employer in the United States needs an Unemployment Account number.  If your a new employer, and didn't recieve yours yet, write new in this field.
  4. Company Name, Address, and Contact Info - Enter your official Company Name, Address, and your contact information in the section
  5. Banking Information - Enter in the Bank Account information on the account of where your payroll will be deducted from.  This information is used to create your checks and pay your employees.  BE SURE TO INCLUDE A VOIDED CHECK
  6. Information on your Employees - Enter the basic information on your employees: current employee count, pay frequency, number of employees who wish to use Direct Deposit, and your current Unemployment rate for your company.  If you are a new employer, write NEW in this box.
  7. Information on You - Print, Sign, and Date the Form.
 Employee New Hire 

You may use this form when you hire new employees.  If you currently have a form that you use, feel free to use that form, as long as the following basic information is included:

  • Employee Full Name
  • Employee Address
  • Social Security Number
  • Marital Status
  • Number of Federal Exemptions
  • IF YOU ARE IN  A STATE THAT HAS LOCAL TAXES: We need to know the School District and Munnicipality of the employee.
  • Pay Rates

The other information on the form is optional.  The more information you include, the more we can return to you in management reports.  For more detail on these field, please contact our office for explainations.

 New Client Checklist 

This form is a checklist showing you the information we can OPTIONALLY enter in our system.  The more information you can give us on each employee will produce better management reports for you.  These optional fields will show up in a variety of reports.

 Payroll Agreement 

THIS IS NOT A CONTRACT. This form lists the services Net Pay will provide for the fee that we charge.  This AGREEMENT is to protect both you, the customer, and Net Pay.

On Page 1:

  • In the Introductory section, write in Today's date, your official company name, select corporation or LLC, write your address, and your Federal ID/EIN number.
  • In Section 2, Check your Pay Frequency, and Initial the "Accept Included Services" line
  • Initial Bottom Right corner of page

On Page 2:

  • In Section 4, Write in your current number of employees, and then calculate what your rate would be in a pay period.  ("Number of employees" x "rate", or minimum charge, whichever is greater).  Initial to confirm AUTO CHECK payment.
  • Initial Bottom Right of Page

On Page 3:

  • In Section 6, Enter the first pay date that Net Pay will process your payroll.
  • Initial Bottom Right of Page

On Page 4:

  • Initial Bottom Right of Page

On Page 5:

  • Sign the Form and have a second employee (If available) sign the form.

On Page 6:

  • Have an Authorized Signer sign his/her signature in both boxes provided. 
  • Fill in your Company name, and sign the bottom of the form.

 

 

 Direct Deposit 

This form is for the employee who wishes to utilize Direct Deposit.  Direct Deposit is a FREE service included in Net Pay's regular service fee.

To fill this form out:

  • Check Start, Stop, or Change in the top Left Corner
  • Write in the employee Information. This includes the Employee Name, Employee Number, and Social Security Number.
  • Write in the Account information on where the monies should be deposited.
  • Attach a copy of a voided check (if checking account) on a SEPERATE sheet of paper.
  • Have employee sign the bottom of the form
